What is Open Discectomy?
Open Discectomy is the time-tested surgical technique for treating symptomatic lumbar disc herniation through direct posterior surgical exposure. Through a midline incision and laminotomy, the herniated disc fragment compressing the lumbar nerve root is identified and removed under direct visualisation. While microdiscectomy has largely replaced open discectomy for routine cases, the open approach remains the preferred technique for very large extruded disc fragments, complex anatomical variations, revision disc surgery or any situation where comprehensive direct visualisation matters more than the slightly larger incision. How the Procedure Works
Anaesthesia & Positioning
General anaesthesia is delivered; the patient is positioned prone on a Wilson frame, allowing safe surgical access to the lumbar spine.
Midline Incision
A midline posterior incision exposes the lumbar paraspinal muscles, which are reflected laterally to expose the lamina and stabilising structures.
Laminotomy
A targeted window is precisely created within the lamina using a high-speed burr — preserving most of the lamina and surrounding stabilising structures.
Disc Fragment Removal
The compressed nerve root is gently retracted; the herniated disc fragment is identified and comprehensively removed under direct visualisation.
Closure & Recovery
Soft tissue layers are closed with absorbable sutures; same-day or next-day walking begins; structured graded physiotherapy guides complete recovery.
