Discectomy with Fusion (TLIF/PLIF)
A herniated disc is removed and the segment is simultaneously fused — providing both nerve relief and durable spinal stability for recurrent disc disease, segmental instability or complex disc pathology.
What is Discectomy with Fusion (TLIF/PLIF)?
Discectomy with Fusion combines comprehensive disc removal with simultaneous interbody fusion — performed as either Transforaminal (TLIF) or Posterior (PLIF) Lumbar Interbody Fusion. After targeted nerve root decompression and disc removal, an interbody cage filled with bone graft is inserted into the disc space; bilateral pedicle screws and rods complete the construct. The procedure addresses recurrent disc herniation, large disc damage with both back and leg pain, segmental instability or spondylolisthesis with concurrent disc herniation. Dr. Hithesh performs discectomy with fusion in Hyderabad when simple discectomy alone would leave the spine unstable or fail to address the underlying pathology.
How the Procedure Works
Anaesthesia & Positioning
General anaesthesia is delivered; the patient is positioned prone on a Wilson frame, allowing safe surgical access to the lumbar spine.
Decompression & Discectomy
Targeted nerve root decompression is performed; disc material is comprehensively removed, preparing the disc space for cage placement and fusion.
Interbody Cage Placement
An interbody cage filled with bone graft is inserted into the prepared disc space, restoring disc height and beginning the biological fusion process.
Pedicle Screw Fixation
Bilateral pedicle screws are inserted into the vertebral bodies under fluoroscopic guidance; titanium rods connect the screws bilaterally for stability.
Closure & Recovery
Soft tissue layers are closed; structured graded physiotherapy and bracing as appropriate guide spinal recovery and successful biological fusion.
Outcomes
Who Needs This Treatment?
- →Combines comprehensive disc removal with durable biological spinal fusion
- →Effectively treats recurrent disc herniation and segmental spinal instability
- →Addresses spondylolisthesis with concurrent disc herniation in single procedure
- →Provides reliable lasting relief from both back pain and radicular leg pain
- →Modern interbody cages deliver durable consistent biological fusion outcomes successfully
- →Outstanding long-term outcomes for complex lumbar disc pathology cases reliably
When disc herniation comes with instability or recurrence, simple discectomy is a temporary solution at best. Combining discectomy with fusion provides the definitive answer — comprehensive decompression plus the durable stability the spine needs.
