What is Revision ACL Reconstruction?

Revision ACL Reconstruction is performed when a previously reconstructed anterior cruciate ligament graft has failed because of re-injury, malpositioned tunnels, biological non-incorporation or hardware-related issues. The procedure involves removing the old graft and any retained hardware, correcting or bone-grafting malpositioned tunnels, and reconstructing the ligament with a fresh autograft — usually patellar tendon, quadriceps tendon or contralateral hamstring. Dr. Hithesh performs revision ACL reconstruction in Hyderabad through arthroscopic technique, restoring stability and giving previously failed patients a renewed pathway back to active life.

Recommended for patients whose ACL graft has failed or re-torn, presenting with recurrent instability and inability to return to sport, supported by MRI evidence of graft disruption.

How the Procedure Works

1

Detailed Re-Evaluation

Previous operative records, imaging and clinical examination reveal the precise reason behind primary graft failure carefully.

2

Hardware & Graft Removal

The old graft and any obstructing hardware are removed arthroscopically while preserving as much usable bone tunnel as possible.

3

Tunnel Correction

Malpositioned tunnels are repositioned or filled with bone graft; new tunnels are then drilled at the anatomic footprint.

4

Fresh Graft Reconstruction

A new autograft is harvested, passed through corrected tunnels and fixed securely with interference screws or suspensory buttons.

5

Closure & Rehabilitation

Wounds are closed; a hinged brace is applied; structured progressive rehabilitation guides the careful return to sport.

Who Needs This Treatment?

  • Re-establishes knee stability after a previous ACL reconstruction has failed
  • Corrects errors in tunnel position made during the original surgery
  • Eliminates persistent giving-way episodes and recurrent secondary injury risk
  • Allows planned return to active life and recreational sport again
  • Tiny keyhole portals leave minimal scarring with quick wound healing
  • Specialist revision experience improves surgical accuracy and final outcome

Revision ACL surgery is humbling work. Until you understand why the first graft failed — tunnel position, fixation, biology, re-injury — you cannot design a reconstruction that holds for the second time.
